Information on how to claim and offers form part of these Terms and Conditions.
The offer is only open to residents of Australia who purchase Map Coffee Bella Capsule Machine models: S04R Red Bella, S04B Black Bella, S04W White Bella (participating models) from any Map stockists during the promotional period. Employees of the Promoters, Suppliers and their agencies (including resellers and other agencies associated with this offer) and their immediate families are ineligible to participate.
Offer commences on 01/12/2012 and ends at 11:59pm AEST on 31/12/2012 (the Promotional Period). All claims need to be received no later than last mail 14/01/2013.
To be eligible to receive the $75 gift card, a claimant must log onto www.mapcoffeecashback.com.au and complete the online entry form providing requested details, including details of the Qualifying Purchase (the serial number of the Participating Product, full purchase receipt/tax invoice number, product model number and retailer of purchase) and upload a scanned legible copy of the original receipt. Or the claimant can print out a completed claim form and mail in with the original receipt to ‘Map Coffee – Christmas Promotion’ PO BOX 422, Moorabbin, VIC 3189 by last mail, 14/01/2013. No responsibility is accepted for late, lost or misdirected mail. Proof of posting is not proof of receipt. The Promoter reserves the right to verify the validity of claims and to disqualify any claimant who tampers with the redemption process or who submits a claim that is not in accordance with these Terms and Conditions.
A consumer whose entry has been deemed valid will be sent the $75 Map branded eftpos gift card within 14 business days by ordinary mail to the address provided on the entry form. It is the consumer’s responsibility to provide the correct address. The promoter is not responsible for any delays in payment or errors due to factors outside its reasonable control.
The card will entitle the bearer to $75 to be used anywhere eftpos is accepted. Gift card cannot be used to withdraw cash. The eftpos gift card will need to be activated online at www.activatemycard.com.au/mapcoffee within 2 months of the eftpos gift card being dispatched. The gift card will expire 12 months from the date of activation. If you have not received your card or require assistance with your card activation, please call 1300 079 267. Please note that a local call fee applies and calls from mobile phones are subject to additional charges.

Any costs in excess of the value of the card (including GST) and all ancillary costs associated with Redeeming the offer is the responsibility of the claimant.
To the extent permitted by law, the Promoter does not accept any responsibility for the goods and services purchased with the card.
To the extent permitted by law, the Promoter and/or the offer Supplier reserve the right to eliminate, add, change and substitute the card for another open loop Gift Card of equivalent value without prior notice provided that they are of approximately equivalent type and value. This does not affect your statutory rights.
By redeeming this offer you are stating that you accept the terms of use of this card.
To the extent permitted by law, the Promoter, the offer Supplier and their agents and distributors accept no responsibility, financial or otherwise, for any liability relating to (a) any inability by the bearer to use the Gift Card properly or at all; or (b) the misuse of the Gift Card.
The Promoter, the offer Supplier and their agents and distributors offer no warranties or representations regarding the advice, instructions, or any other information provided by the venue.
The Promoter, the offer Supplier their agents and distributors cannot replace any lost, stolen or damaged cards.
